How Do I Join the PTA?

In an effort to save paper, we are automating some forms.
Please fill out the form below, for each child you have at Cherry Valley.

This form will automatically go to the Directory Coordinator.
Please remember to press the SUBMIT button at the end of the form. This will take you to the payment button.

IF YOU ARE PAYING BY CHECK, YOU CANNOT USE THE ELECTRONIC FORM. You must go to the office and fill out the paper form.
Membership Pricing: $18.00 (This includes the Membership Directory)
Additional Directory: $7.00

You will receive your 2011/2012 School Directory, when it's completed. The directory is a great way to make play dates with classmates or keep in touch with other classroom parents!
